I have narrowed down the YouTube video playback problem.  

When I thought the video was not playing back, it was actually that the video 
was dimmed so much that I could not see the playback.  

So, it's rather difficult to put into words but, essentially, when a 
non-VoiceOver user stops or starts a video using the on-screen controls, the 
video dims, for a few seconds, which makes the controls easier to see.  After 
the controls are activated, the video brightens to its normal level.

When VoiceOver is enabled, and the controls are activated, the video remains 
dimmed during playback.  It's really that simple.  Strangely enough, 
advertisements that may precede the target video do not have this problem 
because they do not dim during playback.  

This, as I have asserted since the beginning, has to do with the screen 
refreshing when VoiceOver is enabled.

I experience some more issues. To begin with VoiceOver not reading the duration 
of videos on the start screen anymore in the app. It seems to be a graphic 
which VO can’t read.

Two finger double tap to start and stop content from playing seems to work just 
randomely but should be consistent.

Beside VO related issues stop videos in the app from autoplaying seems only to 
work on the start tap. I disabled autoplay but it doesn’t work for videos in 
downloads or watch later playlist. In these lists autoplay is still turned on 
and there’s no button to uncheck it..

And last but not least for me it would be wonderful to have a sleep timer in 
the app itself
