You find the tweet, then swipe up or down (down is faster) until you find "open media". Double tap to bring up the image, on which VO should focus automatically. Now do a three-finger tap to get any description or text VO can generate. You might need to try it a few times. I find that, if I have nothing after three or four tries, I'm probably not going to get anything. Do a two-finger scrub when you're done, and the image will close.
