Well, either the developer of the App would need to do this with their programming, or there is one other possibility.
Usually when a VO enabled app updates a field that your cursor is on, it will automatically be read. So if you disable phone sleeping, touch the heart rate field, then VO should read that field every time it changes. I have done this with the transit app, I touch the line I am taking and it will count down the minutes until the bus arrives. It might be worthwhile contacting the developer. certainly I know I would be tempted to buy such a device if I knew it could give regular updates via VoiceOver and notifications of critical rates.
