I have never experienced these issues. Perhaps do you have speak text under 
mouse enabled and also mouse follows VO cursor? I could see some possibility of 
how those settings would cause the dialog boxes to speak everything when moving 
to the last item on screen. It might be worthwhile creating a temporary user to 
see if you get this overspeaking of dialogs and under speaking of column mode 
in finder.
