Are you using Alex by any chance? If you recall my last message about this issue, it only happens for me if I use Alex as my Voiceover voice. It's strange that Macs 2011 and up don't have this issue but it's really disconcerting that the 2010's do and I really hope Apple fixes this right away. In the meantime, use one of the Nuance voices if you have them. They never crash Voiceover and if you don't have one and need help installing one I can guide you through how to do it.
