For what it’s worth, I experience this occasionally as well. It always occurs after I wake the computer from sleep, and I find that VoiceOver is running, but muted. I am not sure what causes this problem but it is especially difficult to work around. To help, I’ve assigned the “mute VoiceOver” command to a hotkey using the VoiceOver keyboard commander preferences. At least this gives another option if the trackpad gesture doesn’t work. It will not work if you are at the lock screen in the password field, and the command to toggle VoiceOver won’t work on the lock screen at all. Hearing the error tone when you press COMMAND-F5 actually gives you one of the few indications that are available at this point, which is that you are likely in the lock screen meaning you need to get passed it before trying too much else. Once you are passed the lock screen, using the commands to toggle VoiceOver and toggle mute are sometimes effective, but not always. Sometimes if there are no apps that need your attention, you can at least cleanly reboot the system by, say, pressing the POWER button for a few seconds to bring up the shut down dialog, then pressing r for restart.
I am not sure why this occurs, and it may be that it only occurs on some Macs but not others. Would love to hear from others on this.
