Hello E.T., I completely understand your confusion.
As for iTunes continuing to display the Transfer option, I'm almost certain it will be removed in the next major release. In terms of making sure that you have an accessible version of your apps, the only solution, at this point, is to manually download the current version into your iTunes Library, from the App Store on your Mac. And do not sync your iOS apps from your device until you have confirmed that it's latest update is also accessible. Here is a typical scenario I follow: 1. I update to app X on my iPhone. It is accessible and I like the app. 2. In iTunes on my computer, I download app X. So, now I have the same version of app X on both my phone and my computer. Please note that I did not connect my phone to my computer and I do not have automatic downloading or auto syncing enabled on either device. 3. Three months later, when checking for app updates on my phone, I discover that there is an update to app X. So, I update the app. If the app continues to be accessible, then, just as before, I manually update the app on my computer from within iTunes on the computer. However, if after updating app X on my phone, I discover that accessibility has been broken, I delete app X from my phone. Then, connect my phone to my computer and select to copy the app from my iTunes Library, on my computer, back to my phone. This is how I make certain that I always have an accessible version of all of my important apps. Now, in terms of restoring a device: 1. If you elect to restore your iOS device from an encrypted iTunes backup, all of the passwords, network settings, and iTunes Library apps will be restored to your device. 2. If you elect to restore your iOS device from a non-encrypted iTunes backup, only the general settings along with a list of all of your previously installed apps will be restored. By merely restoring the list of your currently installed apps, this means that, assuming you are connected to WiFi, the latest version of each app will be downloaded from the App Store, directly. 3. If you elect to restore your device from an iCloud backup, as in the item above, each app will be automatically downloaded from the app store, directly; meaning the latest version of the app. Please keep in mind that I am writing this based on my personal experience, only, as of the last 3 months or so.
