Go to VoiceOver Utility, sound category.  If it’s set to “System Default” for 
output, then that’s probably because you’re using AirPlay, so either manually 
set VO to the correct device or stop using AirPlay from the menu bar.  If you 
aren’t using the default then VO is trying to be helpful by pointing out that 
it won’t be doing a lot of good going through AirPlay, which is not normally 
what you want.
