I was frustrated that I could only get VO to read image descriptions if I opened the tweet and flicked to the image. Anyways, I went to settings, look and feel and changed in line images from disabled to thumbnail. Now it appears so far that images with alt text are being read in my timeline. I am experimenting so I can't report on any long term experience nor have I tried any of the other options for in Line photos.
