It's not just the internal speakers. It's doing this regardless. Even on
my bluetooth speaker, it's doing it. It's fine everywhere else though
except for ITunes.

Subject: Re: Perplexing problem with ITunes: very quiet playback
Is the volume quiet on just the internal speakers? Headphones? Other
(external) sound cards? All of the above?

I’m pulling my hair out over here trying to figure this out.
I’m running the most up to date version of ITunes, El Capitan 10.11.3, no
beta build. This is the official release of everything.
This is an eluminum Macbook pro 13 inch mid 2012 unit.
Every time I play anything, it sounds ridiculously quiet. I have pressed
command+up arrow and also have manually navigated to the volume slider and
made sure it was up all the way. I do not have audio ducking enabled in
Voiceover preferences.
In the ITunes preferences, I made sure crossfade was disabled, which it
is, and I also made sure the sound enhancer was off. Sound check is also
disabled.
I don’t have this issue on any of my other mac systems. I get that the
speakers in this model macbook naturally were a little bit quiet, in
general, but trust me. They shouldn’t be this quiet. Voiceover is
literally coming out about 3 or 4 times louder than any music. And no.
It’s not how the music was mixed/mastered. Trust me. I’ve done this on
about 7 or 8 different tracks, all of which were by different artists, and
from different albums. It’s not even just my music, nor Apple Music.
Podcasts, movies/TV shows, and even Beats1, are also exhibiting this odd
behavior.
Anyone got any idea?
Chris.
