Yes. I’ve got Voiceover audio routed to Internal speakers, OS X sounds going through the system default card. Bluetooth is off and I’ve reset my Airport Express. The problem is reproducible, which is a little annoying as every time I go to show someone the problem, the computer behaves properly.
