Do you have quick nav enabled? You need to, for this to work.
Secondly, do you have single letter navigation enabled? If so, do you have
a Voiceover Activity for Mail, and if so, did you enable these two options
specifically within the activity?
If this is too much work, then try doing command+VO+H instead of just the
letter h by itself.
See if that works.

I have a similar question. I noticed that when in an email that has
headings and links, the voiceover mode on does not work when you press
H and L as it does in safari. Is this just the nature of the beast?
