Hey Chris. I didn't see the first thing you mentioned about the error you saw, 
but I can confirm about the file upload. I think it's messed up. I sent a 
feature request to apple, everything got sent, but it wanted me to attach 
files. I couldn't figure out how to do this so I skipped that. This kinda 
discourages me to use the Feedback assistant on the Mac. I like iOS 9's 
Feedback Assistant better, all the files are collected and uploaded 
automatically.
