Okay, a follow up to my previous instructions. At the point where you need to 
mouse click the word caps lock in the combo box, There are two things you can 
do to get past this little glitchy part.
First, instead of routing the mouse to the VO cursor with VO command F5, I have 
found here as in other programs that setting the mouse to follow the vo cursor 
in VO settings works better than trying to route the cursors in the app .
So, before attempting the remap, open VO settings with VO f8, and under 
navigation, set “mouse to follow VoiceOver cursor”.
Now go ahead and do the remap, and when you arrow down to caps lock in the 
combo box, just click the mouse either with the trackpad or vo shift spacebar, 
and then uninteract once or twice to get out of the combo box. I just tried 
this several times and it worked consistently for me.

the other way is to also set VO to announce what is under the mouse in the 
announcements tab of the verbosity settings of VO settings. Do this in addition 
to having the mouse track the VO cursor.
Then, in the remap combo box, press VO Space on the combo box to open it up and 
insert will be at the top of the list. Slowly draw your finger down the 
trackpad and VO will announce each key in the list as you go. When you hear 
“caps lock” about the fifth key in the list, stop and click the mouse.
Now exit the combo box by uninteresting a few times and verify the selections.

Again, I tested these two methods out and they worked consistently for me.
I am using VMWare V7, but it should work for V6 or V5 too. (I remember doing 
this same procedure back when I was using V5).
