Amazon appears to be making some bad changes because I'm having trouble with more and more screen readers. What I find works best is to press enter in the edit field after the quantity if I want to add to cart. If I'm using one click I find that using the roter to get to the button is most effective. But I do think they're making some changes so our mileage may vary.
