I had major issues with an attachment yesterday. No matter what I did, nothing would happen. I would bring up the context menu and I tried, open, save as, and save to downloads. Nothing would happen though. I also realized later there were actually two attachments. Finally, the one thing that seemed to work, or I got really lucky, was to use an actual mouse click on the attachment instead of my keyboard to bring up the context menu. I really feel, I finally just got lucky. Anyways, you could try using an actual mouse click. I think you hold down the control key on your keyboard and then click with your trackpad/mouse. Also, I have mouse tracking on. My mouse pointer always follows my voiceover cursor.
