Hi,

I'm not experiencing this with the newest iTunes 11.3.  In the couple versions 
prior to this one, there were VO/Keyboard tracking issues in iTunes which made 
this sort of thing less than perfect to perform.  Make sure QuickNav is off, 
Cursor Tracking is on and try arrowing through the Songs list or a playlist.  
With the newest version, using the arrow key without VO keys should do proper 
navigation.  If this is working, then once the song is selected, press 
VO-shift-m to bring up the Contextual menu,, type a d d to move VO to the "Add 
to Playlist" item, right arrow over to open the list of possible playlists then 
choose the specific playlist followed by return.  I realize that in your 
explanation of the original problem, you were essentially doing this, but give 
this precise method a whirl.
