Unlock the iPhone so you are on the main home screen.

Unplug headphones or external speakers.

Perform a two-finger flick up to engage VoiceOver. It will be engaged even 
though you do not hear it.

Start pressing the volume up button repeatedly to raise the VO volume for the 
internal speaker. iOS manages each speaker volume separately. In your case, the 
speakerphone volume got reset to 0.
