Most of the issues I find in software seem to fall into two buckets. Either the developer used standard controls but failed to label them, or they made custom controls and failed to finish the job by making them accessible (undiscoverable or unusable). The former is easier to fix, the latter can be difficult depending on what kind of crazy UI they thought up.
