Modifying /etc/man.conf shouldn't be necessary, my MANPATH is unset, no /opt entries in /etc/man.conf, and man works fine with MacPorts-installed items. Make sure your MANPATH is actually unset and not empty (env |grep MANPATH), perhaps that is what is causing the issue.
