I was thinking of going with #2 or #4.

A trac ticket is fine. A pull request is also fine if the maintainer has
a github account; you can @ mention them in a PR and they will be
notified of it.
We've done some work recently to enable display of GH usernames as well
as email addresses for port maintainers.
