pleading paper

I have been unable to create a pleading paper template. There is a laTex example at

Thanks in advance for any suggestions.

However it does not work with lyx 2.0.5.1. And it seems needlessly complex. All I want is a template with 28 numbered lines down the left side of the page and a line down the right side. Don't know if this list accepts pdfs but will attempt to attach one showing what I am talking about.
