LyX automatically adds certain packages to the preamble if it spots the need for them, but it only sees one of a set of known "triggers". Best guess, what you were doing didn't hit a known trigger. You could file a bug report. I don't know enough LaTeX to know if there is a fairly concise set of conditions under which it is obvious that array is needed (or whether it is safe to say that any time the user futzes with optional commands for columns, array should be loaded).
