Thanks for getting back to me - sorry I didn't reply sooner, was on holiday.

Yes I think there must be a problem with my installation, though I don't know 
what it is.  I found the place where toolbar settings are stored in OS X and 
they are correct.   FYI that location is:

<lyx_install_dir>/LyX.app/Contents/Resources/ui/default.ui   (or classic.ui if 
you are using that ui)

I will try a few more reinstalls before reporting the bug.
