The errors occur because you have some special (in this case invalid) 
LaTeX-code. You see this when you look in the menu 
Document->Settings->LaTeX preamble.

It is strange that you get this code by creating a new file. The only 
reason I can think of is that you or another user on your PC (maybe 
accidentally) saved a special LyX file as default for all new documents. 
To fix this, open the attached LyX file in LyX and use the menu 
Document->Settings and press there the button named "Save as Document 
Defaults".
Afterwards close LyX, restart it, create a new file and have a look if 
it then works.
