I installed Lyx 1.6.5 and MiKTeX 2.8 and this has fixed the problem. One note: the Lyx/MiKTeX install didn't work unless I uninstalled Adobe Acrobat Reader, restarted, installed Lyx/MiKTeX and then re-installed Acrobat Reader.
