You haven't filled the second column of the cases environment. Without this column, the cases environment doesn't make sense in the mathematical sense it is designed for. Attached is your example file with two possibilities to fix this. Also have a look at sec. 3.5 of LyX's Math manual that you find in the Help menu of LyX.
