Thanks Jürgen,

I've managed to solve this, and will document it on Troubleshooters.Com 
shortly. It's basically what you said, although the examples in 
texmf/makeindex are too complex for the person not understanding the concepts 
of makeindex. The makeindex man page is better suited to the newbie (but my 
web page will be better suited yet :-)
