I'm guessing there's nothing wrong with your LyX or LaTeX setup; you're just
not understanding how to make the bibliographic entries.

The bibliographies internal to LyX must use the "Default (numerical)" option
-- not Natbib or Jurabib (in Document > Settings > Bibliography). The "Key"
field can be anything you want it to be -- it's simply what appears in the
LyX GUI. The "Label" field is what appears in the printed output; if you
leave it blank, it will be automatically numbered when the document is
typeset, or you can enter something else, and that will appear in the
printed output placed between square brackets ("[" and "]"). The text that
appears in the bibliography should be placed immediately after the Label/Key
inset, on the same line. (See the attached, edited version of your
document.)

If you want the author-date method, you can fake it as you were trying, but
it really would be better to create a bibliographical database (on Mac, use
BibDesk.app), and then use either Natbib or Jurabib, following the
instructions in the User's Guide.
