Looking at the *.log file I found the following error:
  ! LaTeX Error: File `preview.sty' not found.

Does this indicate the source of the problem?

I will send a trivial example file together with the 01lyxpreview.tex and
01lyxpreview.log files in a separate email (in case it doesn't make it
through the size restrictions on the list)
