If you right-click someplace open in the main menu bar (to the right of Help), I think you'll get a check-list of toolbars. Select Table. Now when you're in a table you'll have a table-specific toolbar, including buttons to add/delete individual rows/columns. (I know this works in 1.4.4; I think it works in 1.4.2.)

You can hack the stdtoolbars.inc file to make the Table (and also Math) toolbars pop up automagically based on context. At least in 1.4.4, you can get this without hacking by selecting default-autotoolbars.ui as your UI file.
