I've attached a sample document (not sure if it's what you want). To get the block environment, I simply started a new paragraph and selected Block from the drop-down list. To add the block title, I used an ERT inset and put the title (in braces) in ERT. Items were created as usual; to put the first two items inside the block, I nested them.
