My LyX document and the PDF are saved in the same folder, and I'm using the following in an ERT:\includepdf[pages==-]{Questionnaire.pdf}
I have had trouble in the past with the dash to indicate all pages. Try putting the first and last page numbers, e.g. [pages=1-5]
