Probably a silly question, but did you put \usepackage{booktabs} in the preamble?
I'm attaching a small LyX document that contains the example table from page 5 of the booktabs manual (in ERT). Does it compile correctly on your system? If not, what version of LyX are you using, on what operating system, and what is the first error message you get? Also, are you using English or a different language?
By the way, I can display this correctly with both LyX 1.3.7 and 1.4.1svn. I built the file on Windows; hopefully the end-of-line characters won't cause problems if you are using Linux or MacOS.
