This is consistent with how the "emphasize" and "bold" buttons work.
If you have a selection, the selection is updated.
If the cursor is in the middle of a word, the whole word is updated.
This is an optimization, we don't have to make selections
in the common case of emphasizing a single word.
If none of the above applies, the cursor itself changs, so if you
start typing (without moving the cursor first) then the new setting
applies to the new text.
