I added a "\newpage{}" immediately before (without it said the Index was on
previous page, 57) and used that "\addcontentsline{toc}{chapter}{Index}".

I switched to "Two-sided document".
My table of contents "Index" entry points to a blank page 58 (even left
side) but my Index is on the next page 59 (odd right side).
(If I don't have a newpage before my index, then my table of contents
shows page 57 for the Index, but really it is content from previous
chapter.)
My fix is:
Insert a page break above immediately before (Layout -> Paragraph ->
Lines & Pagebreaks tab -> Page Breaks Above) my ERT which contains:
\cleardoublepage{\addcontentsline{toc}{chapter}{Index}}
Then that is followed by the Index (Insert -> Lists & TOC -> Index List).
