Not sure this applies to all platforms (and not sure what you're running on), but on Windows, htlatex needs either to have some environment variables set or to find a file named tex4ht.env (or both -- don't recall). Does it look for environment variables when you run it from a shell, and if so does it get the same environment variables when LyX calls it?
