The indexing system is well explained in "The LaTeX Companion 2nd Edition" but not in LyX's docs :-(. I'll add an instruction to the docs if I find the time.
If the LyX docs both referenced Chapter 11 of TLC2 and stated that all the markup should be entered in the LyX index entry box it would suffice (at least, for those of us who have TLC2 here on the desk).
