Herbert,

  Yes! I discovered the problem and was just going to write. In Uwe's
example I saw, "use amsmath 1". In my file it was "use amsmath 0". Changed
my file and it compiles.

  It was not that I forget; I didn't know to do add the amsmath package in
the preamble. This was not "intuitive" for me. :-)

  Whew! This was a major stumbling block and I learned a lesson not soon
forgotten: use the amsmath package if there will be equations in the text.
