I would be surprised if there were no hints on Herbert's page.

Anyway, this goes into the document preamble (Layout->Preamble), and you
use it as '\myNote{some text}' in ERT mode (Ctrl-L).
Even better: '\mynote{' in ERT, then 'normal text', then the '}' in ERT
again.
