I don't understand this: if the converter program for pdf is set to
tex2pdf, LyX doesn't run any more makeindex.

What do you mean by "error message" ? When you ask for View->DVI,
the index is simply not formatted as you want it. As it is a
standalone prt in the document, it's OK for the layout as far as
the PDF is OK at the very last run.

Checked here that setting makeindex option either in tex2pdf config file
or as a command line arg in the Preference->Converter menu overrides
the hardcoded lyx command.
