A bit off the track, but IMHO this is the single most annoying thing anyone can do in an index. I would love to have an option in the index generator that says A and B are equivalent, so both A and B get a list of the references to either.
I agree, but I can't see a way to do it in makeindex. Having said that ...
The single and most annoying thing anyone can do in an index is not provide an index. Call me anal retentive, but I even want an index in a novel.
