Would you like a sneaky, kludgy way to do it? I did it by putting title and author as you would expect (and in your case you'd add the word "by to the author, followed by an ert //. Then I did my magic, including all the rest of the stuff in a single paragraph formatted in the Date environment, with linefeeding controlled by // and/or //[1.0in], etc. Sometimes I'd need to put a nonbreaking space in front of the // to kill errors. It was a kludge, and it was sneaky, and it totally defeated the purpose of the Date environment, but the result was a very nice looking title page.
