Sorry for auto-replying, but I've managed to produce these indexes from a minimal example (Memoir and selecting multiple indexes from the lyx menu). Do not lose time on this. The problem seems to come from other options in the preamble, which clash with the index. I'll try to find out exactly what interferes and if I'm successful, I'll post the result in reply.
