Almost every page. There's a command defined in the doc's preamble, and before compilation, the person's name is substituted for a token in the name. That command prints in the page footers on every mainmatter page that isn't the first page of a chapter. The footer says "Prepared exclusively for Scott Kostyshak", discretely, in a red/maroon bar.
