Strange. But OK, another shot in the dark:
If you open lyx file in some plain text editor (like vim/notepad/...)
and you look at the header, you should see line starting with "\origin"?
What does the whole line says?

Pavel,
For what it's worth, I created a small test file (on Linux Mint) with a
single image and saved it. The \origin line you asked about says
"\origin unavailable". The lines inserting the image read
\begin_inset Graphics
filename /home/paul/Documents/OSS License.png
\end_inset
and the image loads properly when I open the document. Now we need Ehud
to give a similar reply, to which we can compare this.
