The version of hyperref.cfg you attached matches the one I copied from another machine, and it does *not* solve the problem. I'm attaching a DVI that shows the spurious lines. It was produced by LyX 1.3.7; LyX 1.4.1 produces identical output.
Unfortunately, I failed to make note of which packages updated when I ran the MiKTeX update applet yesterday. Do you have any idea what might be causing this?
