Using the AMS document classes, a file including the "Current address"
and "Dedication" layouts causes a Latex error due to a double
declaration of 

\newcommand{\lyxaddress}[1]{
\par {\raggedright #1
\vspace{1.4em}
\noindent\par}
}

This arises because the file amsdefs.inc copies the Address style for
the definition of a number of styles, such as "Current address",
"Dedication", "URL" and so on. Unfortunately, the universally included
file lyxmacros.inc redefines "Address" to include
\newcommand{\lyxaddress}... in the preamble, with the result that it
will be repeated whenever one or more of these styles is used.

The remedy is probably to remove the input of lyxmacros.inc from the
AMS layouts; this would also remove the unneeded "Right address", but
also "Lyxcode", which some might miss.
