I don't understand what you mean. Can you please provide a minimal 
example .tex file and the resulting .lyx file?

Again: Example please. The \appendix command should not be moved to a

Again, please provide an example .tex file.

This is missing functionality in lyx2lyx

The layout files are wrong. Maybe they need to be converted for 1.4?
