Here it is. I didn't know that "cvs diff" creates a patch against the currently checked out version instead of against the head.
Highlights: - translates TeX units to Docbook units - translates \textwidth etc to "%" - uses <mediaobject> for XML - fixes spurious '>' chars for SGMLtools
